Revolutionizing Epilepsy Surgery with AI-Powered Semiology Analysis

In the field of neurological disorders, epilepsy remains a significant challenge for millions globally. With over 70 million people affected worldwide and approximately one-third unresponsive to medication, surgical interventions targeting the epileptogenic zone (EZ) offer hope. However, the current success rates hover around 50%-60%, partly due to inaccuracies in identifying the EZ. Researchers at Stevens Institute of Technology have explored the potential of large language models (LLMs) like ChatGPT to enhance seizure semiology interpretation, improving pre-surgical decision-making. By comparing AI performance with human experts and developing a specialized model called EpiSemoLLM, they aim to bridge inconsistencies across epilepsy centers and refine localization accuracy.

Pioneering Research: Bridging Gaps in Epilepsy Diagnosis

In an era marked by technological advancements, researchers at Stevens Institute of Technology are reshaping epilepsy care through artificial intelligence. The study focused on leveraging LLMs to interpret seizure semiology more effectively. Conducted under the guidance of Assistant Professor Feng Liu, the research involved surveys completed by five certified epileptologists. These experts evaluated 100 descriptions of seizure symptoms, while ChatGPT performed the same task. Results revealed that ChatGPT matched or surpassed human accuracy when predicting EZs in commonly affected brain regions such as the frontal and temporal lobes. However, human expertise remained superior in less frequently involved areas like the insula and cingulate cortex. To further enhance these findings, Liu's team developed EpiSemoLLM, a custom-built platform hosted on a Stevens GPU server. This innovation could serve as a critical assistant during pre-surgical evaluations, ensuring more precise EZ identification.

This breakthrough underscores the importance of collaboration between humans and AI systems. While LLMs provide unparalleled consistency and speed, human intuition and experience remain indispensable in complex medical scenarios.
